What it does
Converts a heparin order into a pump rate in mL/hr and a bolus volume. This does the rate math only; it is not a titration protocol.
The formulas
concentration = units in bag ÷ mL; rate mL/hr = ordered units/hr ÷ concentration (for units/kg/hr, multiply by weight first); bolus mL = bolus units ÷ concentration.
Worked example
25,000 units in 250 mL is 100 units/mL. An order of 1000 units/hr: 1000 ÷ 100 = 10 mL/hr.
Good to know
aPTT-based titration follows your facility’s protocol, which this tool does not replace.
Study calculations only, not dosing recommendations. Real medication orders and administration require the appropriate clinical checks and facility protocol.
